When you Create Digital Forms you can select from different categories of Answer Types to determine how the worker responds to each question in your form.

Display a text field for a worker to provide a free-text response. You can also include a URL in your text field question, which is displayed as a clickable link to workers in the Samsara Driver App. |
Number |
Worker must write in a number. This option accepts numerical input only. |
Date / Time |
Worker must enter the date and/or time. |
Instructions |
Enables you to provide instructions to workers without requiring a response. This field ensures workers receive all necessary information to complete the form accurately. If your instructions are lengthy, they will be truncated with a See more button to ensure workers read the full text before submitting the form. |
Media Instructions |
Enables you to add media, such as images and videos, to guide workers in completing forms. Videos must be 15 MB or less, and images must be 3 MB or less. |
Media |
Worker uploads a media file from their library or takes a photo through their device's camera. Workers can upload photos, or scan a document through their camera. For workers who fill out form on the Samsara dashboard, they can also upload PDFs and documents. |

Person |

Some forms are designed to include a person (such as a worker or admin), inspect assets (such as vehicles, trailers, or equipment), or include a geofence to provide location information. When you create a form you can add a Person, Asset, or Geofence field option through its respective answer type. This enables the worker to link that form and any related issues directly to a person, an asset, or a geofence in Samsara.
For example, if a worker needs to fill out an Accident Report form, a worker would be able to connect the associated asset involved in the accident, the associated persons within your organization that are involved in the accident, and the location of where the accident took place.

Asset answer type: Enables workers to connect an asset to a form through an Asset field.
Each form can only have one Asset field.
Based on how you configure the field for what asset types can be selected, Connected Workflows pulls the list of assets by name from the Assets Summary (Overview (
) > Assets) in your Samsara dashboard.

Person answer type: Enables workers to connect a person to a form through a Person field.
Each form can have multiple Person fields.
Based on how you configure the field for Selectable People, Connected Workflows pulls the list by role from Users & Roles (Settings > Users & Roles) in your Samsara dashboard.

Geofence answer type: Automatically associates the form with the nearest geofence when the worker opens the form, and enables workers to correct the geofence location if necessary.
Each form can only have one Geofence field.
If you select Manage Addresses/Geofences, a separate tab opens to Address/Geofences settings in the Samsara dashboard.
When the worker selects an Asset, Person, or Geofence question in the form, it displays the field types you selected for a worker to choose from. If the worker is offline or cannot find the correct selection in the list, they can also manually type in the name. This will not alter or add a new entry into your Samsara dashboard settings.
From the Samsara dashboard you can access your Issues Inbox and filter issues by asset number to view all the issues associated with that asset number. This helps you gain visibility into the status of an asset's previously submitted open issues, and can reduce duplicate issues.
