Digital forms digitize your paperwork and enable instant, paperless access to all your forms, such as equipment inspections and maintenance records. Within the Template Builder, you can create, manage, and delete your forms, as well as view all existing digital form templates. Answer Types determine how the worker responds to each question in your form and include the following options:

Display a text field for a worker to provide a free-text response. The max character count for the text field is 1,024 characters. |
Number |
Worker must write in a number. This option accepts numerical input only. |
Date/Time |
Worker must enter the date and/or time. |

Worker uploads a media file from their library or takes a photo through their device's camera. |
Signature |
Worker signs the screen. |
Person |
Lists people from the Samsara dashboard Users & Roles settings, which the worker must choose one from. To learn more about this field, see Asset and Person Fields in Connected Workflows |
You can build a digital form from scratch, or you can Create a Digital Form Using a Pre-made Template. To create a digital form from scratch:

From the Samsara dashboard, select Forms > Templates.

Select + Create > Build a Template.

Name your form in the Untitled Form field.

Add a section to the template with + Add Section.

Title your section in the New section field.

Type your question in the Question column.

Select the radio button to choose the Answer Type.

Click the Required toggle if the worker must answer the question.

To enable workers to upload images—like documents and real world pictures—that automatically fill in form fields based on the image content, select the Media Answer type, then Enable Autofill with AI.
Your organization must have access to AI Visual Intelligence to use this AI feature.

Add and link fields you would like auto-filled by AI with the Autofill from drop down.
Supported field Answer Types that AI Visual Intelligence can populate include Asset, Text, Number, Date / Time, Multiple Choice, and Check Box fields.

Add field descriptions to each link field to help guide the AI on what specific data to extract from the image.
Be as detailed as possible when you write descriptions because they are fed to the AI to improve prompt accuracy. You can tweak descriptions to increase the accuracy of the extraction over time.

Multiple Choice or Check Boxes Answer Types only: If you want to add follow up actions based on how a worker responds to a Multiple Choice or Check Boxes answer type question, click If answer is..., choose which answer to create the follow up action for, and then select + Add action.
You can include one or more of the following follow up action options:

Creates an issue in the form that the worker must fill out and submit.
Submitted issues can be viewed in the Connected Workflows Issues Inbox.
Require Note
Worker must type additional information.

This option is available to Premier tier only.

Add another question to the section with + Add question.

(Optional) To enable workers to enter structured information in a table with dynamically added rows to fill out forms, select + Add Table.
Tables can be especially useful for forms that rely on structured lists, such as witness details during an accident report with multiple witnesses, or in audits and inspections where multiple defects, maintenance tasks, or compliance checkpoints need to be logged efficiently.

Title your table in the Table field.

If you clicked the Enable Autofill with AI toggle while you created a section in step 4, you an also toggle on the Autofill table from drop down option.
This option enables you to link fields that AI auto-fills, based on the image a worker uploads.
Your organization must have access to AI Visual Intelligence to use this AI feature.

Define each column.

Select the Text button to choose the Answer Type.
Supported answer types for tables are Multiple Choice, Check Boxes, Text, Number, Date / Time, Media, Signature, and Person.

Click the Required toggle if the worker must answer the question.

Add another column to the table with + Add question.

Add additional tables or sections with questions, as needed.

Save. View all your saved templates through Forms > Templates.
All saved templates are instantly viewable as digital forms by all users in your organization in the Samsara Driver App.

The Template Library provides a collection of pre-made form templates that you can use to streamline your workflow setup. Instead of creating forms from scratch, you can select and customize templates that align with your specific use cases, such as safety inspections or equipment tracking. This reduces setup time and makes form creation more accessible, particularly for new users.

From the Samsara dashboard, select Forms > Templates.

Select a pre-made template.

Click Create in the top right corner and select Customize a pre-made template.

Locate the template in the library.
To find a specific type of template, use the search or filters to display templates by:

Industry: Industry-specific forms such as Construction, Utilities & Energy, Government and more.

Use case: Safety, Equipment, Workforce, Transportation.

Regulatory body: OSHA, DOT.

Customize the template.
Modify the template as needed to fit your organization's requirements.

Note
If you reach the template limit defined in by your Samsara license (Essential: 5 templates, Premier: 25 templates), you must delete an existing template before saving a new one.
