The fuel receipt form enables drivers to submit fuel receipts from the fueling station in a standardized form on the Samsara Driver App. This is a useful alternative for organizations that need to track fuel purchases, but cannot use Samsara fuel card integrations or Fuel Purchases API. Submitted fuel receipt forms flow directly into the Fuel Purchases Report, and skips the time consuming process of manually downloading a CSV of fuel receipts to upload into the Fuel Purchases.
The fuel receipt form supports the AI Extract feature, which enables drivers and users to pre-fill fields on the form by uploading of a photo of the fuel receipt on the form, and reduces the time for manual entry. Drivers use the same workflow for completing a fuel receipt form as they would completing a digital form on the Samsara Driver App. After the driver fuels their vehicle, they can navigate to the Forms tile and then select the Fuel Receipt form to fill it out.
The Fuel Receipt form can also be submitted manually on the Samsara dashboard through the Connected Workflows Submissions dashboard. For more information on how to complete a digital form in the Samsara dashboard, see Complete a Digital Form.
The fuel receipt form is a pre-made digital form template in Connected Workflows that you send to your drivers. Each organization can only have one of these pre-made fuel receipt templates and the questions on the template are not editable. To create a fuel receipt template, see Create a Digital Form Using a Pre-made Template for instructions, and select the Fuel Receipt template.

Once a driver submits their completed form in the Samsara Driver App, you will be able to see the submission in the Fuel Purchases Report and the Connected Workflows Submissions Dashboard.
