Routing Forms are purpose-built forms for route-based workflows such as proof of delivery (POD), bill of lading (BOL), and other common transportation workflows. When assigned to a route stop, a Routing Form displays as a task for the driver to complete at that stop. Routing Forms provide a more structured and scalable method of standardize route tasks, and serve as an alternative to driver documents. They are designed for transportation and warehousing organizations using route stop tasks, TMS integrations, or manual delivery documentation.

You can build a Routing Form from the Samsara dashboard, through the Template Builder (Forms > Templates). For step by step instruction, see Create Digital Forms.
When building a form, category selection is required. To access the Routing-specific experience, select Routing from the Category drop-down menu.
Routing Forms are integrated with Route Stops and you can assign them in the following ways:
To manually assign a Routing Form through the Samsara dashboard, perform the following workflow:

From the Samsara dashboard, select Dispatch (
) > Routes.

Select a route.

Choose a specific stop on the route, then click View Details.

Under Forms, select + Add Form to add an existing Routing Form template.

After the form is assigned, it is available to the driver when they reach the stop in the Samsara Driver App.
