Digital forms digitize your paperwork and enable instant, paperless access to all your forms, such as equipment inspections and maintenance records. Within the Template Builder, you can create, manage, and delete your forms, as well as view all existing digital form templates. To create a digital form, perform the following workflow:

From the Samsara dashboard, select Forms > Templates.

Select + Create.

Name your form in the Untitled Form field.

Add a section to the template with + Add Section.

Title your section in the New section field.

Type your question in the Question column.

Select the radio button to choose the Answer Type.
Answer Types determine how the worker responds to your question and include the following options:
Answer Type
Description
Multiple Choice
Display several possible answers which the worker must chose one from.
Check Boxes
Display several possible answers which the worker can choose multiple from.
Asset
List assets from the Samsara dashboard Assets Summary, which the worker must choose one from.

Text
Display a text field for a worker to provide a free-text response.
The max character count for the text field is 1,024 characters.
Number
Worker must write in a number. This option accepts numerical input only.
Date/Time
Worker must enter the date and/or time.
Photo
Worker uploads a photo from their library or takes a photo through their device's camera.
Signature
Worker signs the screen.
Person
Lists people from the Samsara dashboard Users & Roles settings, which the worker must choose one from.

Click the Required toggle if the worker must answer the question.

Multiple Choice or Check Boxes Answer Types only: If you want to add follow up actions based on how a worker responds to a Multiple Choice or Check Boxes answer type question, click If answer is..., choose which answer to create the follow up action for, and then select + Add action.
You can include one or more of the following follow up action options:

Require Photo
Worker must include a photo with their response.
Create Issue
Creates an issue in the form that the worker must fill out and submit.
Submitted issues can be viewed in the Connected Forms Issues Inbox.
Require Note
Worker must type additional information.
Ask Follow Up Question
Asks the worker an additional question.
This option is available to Premier tier only.

Add another question to the section with + Add question.

Add additional sections with questions, as needed.

Save. View all your saved templates through Forms > Templates.
All saved templates are instantly viewable as digital forms by all users in your organization in the Samsara Driver App.
When you delete a form, you can no longer see or edit that form from the Template Builder, and workers can no longer view or submit that form in the Samsara Driver App. However, you can still view and export historical data from a deleted form through the Submissions dashboard.
To delete a digital form, perform the following workflow:

From the Samsara dashboard, select Forms > Templates.

Select the more actions ( ••• ) menu of the form you would like to delete.

Delete the form.
